Creating Failure Records

When failure tracking is set up in APM, failure records are created automatically in response to alarms and when users enter indicator readings as “fixed during inspection”. However, you can also create failure records from scratch, when linking work requests to work orders, when planning work order tasks, when working on checksheets and inspection reports, or when reporting activity for a work order task. You can also link work documents to existing failures. The topics in this section explain the different ways to create failure records.
