Requesting a Failure Mode Review for an Indicator

When reviewing indicator readings on a checksheet or inspection report, you might see a problem, for example, asset degradation is occurring faster than expected. You can request a strategy development analysis for the indicator’s asset.
Note: The indicator type controls whether a failure mode review is available for its indicators. It also controls whether or not the indicator must be referenced on an action plan.
When creating a review request, link it to a new or existing project that you can use to track the review process. When you are ready to begin the review, process the request. Alternatively, you can cancel requests and reopen processed and canceled requests.

To Request a Failure Mode Review for an Indicator

1.
Do one of the following:
•
On the Checksheet window, select the Readings view, Readings tab
•
On the Inspection Report window, select the Inspection Report view and the Readings tab

Right-click the processed reading and click Request Failure Mode Review.
Tip: You can also open a failure mode that references an indicator. In the Implementation view, Primary Action Plan or Secondary Action Plans tab, double-click the indicator to open the Indicator window. Click the Tools menu and then Request Failure Mode Review.
The Request Strategy Development Analysis dialog appears. For example:

Do one of the following:

To link to a new project, select Create a new project. Select a project type from the list.
•
To link to an existing project, select Link to an existing project. Click the browse icon () to select the project and click OK. The project’s number, title, and status are displayed.

Select the type of analysis from the list.

Provide information about the requested analysis:
•
Asset to be analyzed
•
Employee assigned to be the project manager
•
Requested start and completion dates – These become the planned start and completion dates in the analysis
•
Estimate of the time required to perform the analysis
•
Comment

If you are requesting a review for an indicator that is not associated with an action plan, you can identify an analysis, failure mode, or both. These options appear at the bottom of the Request Strategy Development Analysis dialog:

Click OK to save the request and close the dialog. The project is created.

You can view the review request and project by double-clicking the reading to open the Indicator Reading window. Select the Follow-Up tab and then the Review Requests tab. For example:
Tip: You can also view and create failure mode review requests in the Indicator window. Select the Usage view and Review Requests tab.

To open the project, double-click its icon. Work with the project, as required. For more information, see Creating Projects.

To Process a Review Request

1.
Open the Indicator Reading window. Select the Follow-Up tab and then the Review Requests tab.
2.
Select the request, click the Selected list and then Process. In the confirmation message that appears, click Yes. The failure mode that references the indicator appears. The request status changes to “Processed”.
3.
Review the failure mode, as required.

To Cancel or Reopen a Review Request

1.
Open the Indicator Reading window, select the Follow-Up tab, and then the Review Requests tab.
2.
Right-click the request and click either Cancel or Reopen.
The request status changes accordingly.
