Creating a Design FMECA

The first step in recording a design FMECA in APM is to create the analysis from the Site window. In the process, you will select the asset type to be analyzed. Optionally, you can select an analysis type to provide default properties, the analysis’ parent in the hierarchy of FMECAs, and its associated study. This section explains how to create a FMECA and includes links to more detailed explanations of some steps.

To Create a Design FMECA from the Site Window

In the Site window, select the Strategy Development view, the Design tab, and the Analyses tab. By default, this tab lists FMECAs that have been developed for this site and above. You can filter the display by selecting a different option at the bottom of the tab:

Click New. The New Design FMECA dialog appears:

The analysis number is assigned automatically. You can change the number, but it must be unique for the site.

Select the asset type to be analyzed.

The analysis title is automatically set to “Asset type - Design FMECA”. You can change it as needed.

Select an analysis type. Only those that support design FMECA are included in the list.

Select the parent FMECA in the hierarchy. Or you can click the browse icon () to select the parent.

Select an SDA study to associate with the analysis, if appropriate. Study references are used to group, filter, and order analyses.

Click OK. The Strategy Development Analysis window appears. The Facilitation view, Info Worksheet tab is shown.

To set the general properties for the analysis, select the Properties view. The General tab displays information about the analysis. For example:

The Design Details tab displays the analysis type, if one has been assigned to the analysis. You can select an analysis type from the list to set default options.

You can select a different asset type, if necessary.

The parent is the analysis’ parent in the hierarchy of FMECAs.

If appropriate, select a change set from the list. This is usually a change authorization or change set number from a Management of Change (MOC) system.

Select a study to associate with the analysis, if appropriate. Study references are used to group, filter, and order analyses.

If required, enter reference information, for example, the number for an external document.

Review the planned start and completion dates for the analysis and change them if necessary.
Tip: If the analysis was created when an analysis request was processed, the planned start and completion dates are copied from the request.

On the Project tab, you can add the analysis to a reliability project and associate the analysis with a project task. Reliability projects are typically used to organize and track analyses and objects. They usually use project tasks rather than work orders.

On the Operating Context tab, you can record the operating context statement for the asset. For more information, see Recording the Asset’s Operating Context on a Design FMECA.

On the Analysis Team tab, you can record information about the analysis team members. For more information, see Adding Team Members to a Design FMECA.

Select the Analysis Description tab to record additional information about the analysis.
Tip: If the analysis was created when an analysis request was processed, the Analysis Requests tab displays the request.

The Checklist tab displays tasks for your organization’s analysis projects. For more information, see Working with the Design FMECA Checklist.

Use the Meetings tab to record team meetings. For more information, see Recording Team Meetings on a Design FMECA.

The Comments tab helps the analysis team keep track of their work in analyzing assets. For more information, see Entering and Reviewing Comments on a Design FMECA.

Select the Documents tab to browse for standard documents that are relevant to the analysis.

While the analysis progresses, you can attach documents to it using the Attachments tab. For information about attaching a file, folder, note or URL to an analysis, see Adding Attachments to Objects.

Select the Analysis Options tab.

Specify the sites where the analysis will be available for copies. You can choose:

All sites
•
This site only
•
This site and its descendants only
•
Not available for use

Select the Failure Mode Options and Risk Options tabs to change the default settings for the analysis. For more information, see:

On the History view, Status tab you can change the Started on date for the analysis.
Tip: As the analysis progresses, you can select the Analysis Audit Info tab or the Asset Audit Info tab to monitor the status of the analysis and its assets.

Select the Copy Requests tab when you wish to view failure mode copies for which the analysis was the source or target. For more information, see Copying Design FMECA Failure Modes.
