Source Possible sources are: • Indicator Alarm Acknowledgment: The indicator that was acknowledged is listed, along with the work order task (if any), indicator reading, and the date and time at which the acknowledgment took place. • Work Order Task: When a failure is created from a work order task, the task number and title are recorded. • Work Request: When a failure is created when a work request is linked to a work order or when an indicator alarm is acknowledged with a work request, the work request number and title are recorded. • Checksheet: When a failure is created from an indicator checksheet, the checksheet number and title are recorded. • Reading: When a failure is created from a reading, the indicator reading or measurement point reading is recorded. • Inspection report: When a failure is created from an inspection report, the report title is recorded. • Scratch: When a failure record is created by clicking New in the site’s or asset’s Performance Management view, Failures tab, the failure is created “from scratch”.
