Type of RCA

There are two types of RCA analysis: • Reactive. A reactive analysis is performed to determine why a failure occurred with the objective of preventing future occurrences of the problem. Reactive RCA analyses are typically created from a failure record. • Proactive. A proactive analysis is performed to prevent occurrences of potential failure. The objective is to determine what could cause the failure to occur and take the necessary actions to prevent it.
